Waiver *

By typing my name below, I give permission for my child(ren) named above to participate in the Children's Ministry at New Spring Church on Sunday mornings. These programs are designed to help children encounter God through crafts, drama, small group learning, and worship. I hereby release New Spring Church, its staff, and volunteers from responsibility and liability for any injury or illness that my child(ren) may sustain during this ministry. In the event of any emergency, I expect to be contacted immediately. I give my permission to the leaders of this function to authorize any treatment deemed necessary by a licensed physician due to accident or illness during this activity.
